Submit Your Camper’s Required Health Forms
Once you've reserved your spot, it’s time to complete the final steps to ensure a safe and seamless camp experience. We use CampDoc.com, a secure electronic system, to gather and manage health information for all campers. Here’s everything you need to know:
LOOK FOR AN EMAIL FROM CAMPDOC
Once registered, you’ll receive an email from CampDoc inviting you to create an account. Follow the link to complete your camper’s health profile and upload all required health records. This information must be submitted three weeks before camp starts (CampHERO sessions are due June 1). Health forms must be submitted by the deadline. Missing the deadline will result in cancellation of your camp reservation without a refund.
HEALTH DOCUMENTATION REQUIREMENTS
For campers staying 4 nights or more:
- Health History
- Physical Exam (signed by a licensed physician, PA, or RN within 24 months of camp start date)
- Immunization Record or Waiver
For campers staying 3 nights or less:
- Health History
- Immunization Records or Waiver are optional
Buddy Requests
Campers can request up to three buddies in CampDoc, and we’ll do our best to place them in the same sleeping area for the week.
